    Responsibilities

    ROLE & RESPONSIBILITIES:

     

    Exceptional opportunity for an experienced Social Worker to work in designated hospital(s) to increase organ and tissue donation within our service area. Responsibilities include providing specialized support intervention services for newly bereaved families of donor-eligible patients including assessment, support and referral information. Counsels families regarding brain death and the process of donation and obtains consent from family for organ & tissue donation after explaining donation options. Collaborates with LiveOnNY staff on site to work as a team in an effort to provide support to the family and effectively plan for the case.

    Other responsibilities include participating with hospital staff in the development and implementation of strategies, policies and procedures to increase organ and tissue referrals and donation. Coordinating and providing donation education to departments in selected hospitals and performing medical record reviews. May serve as a First Responder on new organ referrals collecting basic patient information from the patient’s chart. In the absence of a Clinician onsite, compiles information such as patient/donor condition, chief complaint, medical history, vital signs and lab values.

     

    Qualifications

    QUALIFICATIONS:


    The successful candidate will possess 3-5 years of experience working with the dynamics of grief and bereavement.  Previous experience in a trauma/critical care setting helpful.  MSW preferred.  Excellent communication skills required. Use of personal car is required. Essential to the position will be a belief and understanding of the organ and tissue donation process. Bilingual a plus.
