Responsibilities

PRIMARY ROLE DESCRIPTION

 

The Case Development Coordinator (CDC) role serves as a primary clinical and operational point of contact for referring donor hospitals and supports timely, accurate communication to facilitate potential donor evaluation and management and case development activities.

 

All Case Development staff must maintain readiness to be deployed, at LiveOnNY’s discretion, to perform hospital-based services in support of donor evaluation, donor management, continuity of care, surge response, or operational needs.

 

The role may involve off-shift work and requires proficiency in independent, self-directed task execution while collaborating virtually and in-person with other LiveOnNY personnel and donor hospital medical care and administrative personnel. All work assignments must remain aligned with LiveOnNY policies and operational needs.

 

The CDC operates in a high-volume, call-based contact center environment and performs continuous incoming and outgoing donor hospital communications and referral intake providing advanced evaluation and management, rapid disposition assessment, escalation pathway execution, and real-time guidance to other Case Development staff. The CDC also provides recommendations prior to physician or leadership escalation, and supports continuity, regulations, SOPs and efficiency across concurrent cases and systems demand.

 

Bachelor’s degree is preferred. Clinical background is required, including a nursing degree or equivalent clinical education, or relevant clinical experience consistent with Case Development requirements.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

  • Perform all essential duties of the Case Development Coordinator II roles, including high-volume call-based referral intake, structured clinical review, documentation, communication, escalation, and structured handoff.
  • Apply advanced critical thinking and experience to rapidly assess potential donor evaluation and management disposition and trajectory using imminent death criteria, clinical indicators, and evolving clinical information.
  • Identify gaps, synthesize incomplete, conflicting, or time-sensitive data to determine appropriate disposition, escalation timing, and prioritization within established pathways.
  • Serve as an expert escalation resource for donor evaluation and management decisions prior to physician or senior leadership escalation.
  • Identify, assess, and lead escalation pathways during assigned shifts to support timely, accurate progression of donor evaluation and management activities.
  • Identify key clinical assessment techniques that may indicate concordance with withdrawal of care goals of care; transition from life-supporting or life-prolonging curative efforts to palliative symptom management and potential withdrawal of support.
  • Support colleagues and leadership in real-time operational decision-making, including surge management, workload prioritization, and resource alignment within the Case Development function.
  • Anticipate downstream operational impacts and communicate proactively with collaborating departments.
  • Collaborate and Communicate with LVNY colleagues, leadership, subject matter experts and donor hospital medical care personnel to assess potential donor status and clinical indicators.
  • Provide clear, timely guidance to Case Development staff regarding escalation, prioritization, and case progression.
  • Coordinate timely escalation and notification to Family Engagement, Hospital Services, Allocation and Placement, Clinical Leadership, Medical Director, and other subject matter experts as appropriate.
  • Maintain closed-loop communication to ensure continuity and system-wide situational awareness.
  • Participate as directed in quality initiatives related to documentation, communication, and escalation standards.
  • Participate in quality reviews, audits, and case evaluations in collaboration with department leadership. Development
  • Participate in structured training sessions, simulations, and case-based learning for Case Development staff.
  • Participate in the development of competency frameworks, evaluation tools, and proficiency standards for Case Development roles.
  • Participate in the development of educational materials addressing changes in policy, SOPs, or practice. Management
  • Utilize LiveOnNY call center platforms, telephony systems, CRM tools, electronic donor records, and reporting dashboards to manage and oversee high-volume, call-driven workflows.
  • Ensure documentation accuracy, completeness, and regulatory compliance across assigned cases and shift activity.

Qualifications

MINIMUM REQUIREMENTS

 

Nursing degree or equivalent clinical education, or relevant clinical experience consistent with Case Development requirements.

 

Minimum of two (2) years successful performance in the Case Development Coordinator role (or equivalent experience as determined by LiveOnNY).

 

Demonstrated mastery of donor evaluation and management pathways and escalation frameworks.

 

Strong understanding of and commitment to the organ and tissue donation process.

 

Bachelor’s Degree is preferred.
